Financial responsibilities for your health care
To find out if our physicians are participating providers with your insurance carrier, please contact your insurance company prior to your visit. It’s important that you understand our Financial Policy and your payment responsibilities so that we can provide you with the highest quality health care and service.

Financial Policy
- Payment is due at the time of service, to include co-payment and deductible.
- We accept cash, checks, Visa, MasterCard, Discover, and American Express.
- We are participating providers with traditional Medicare and Private Fee-for-Service (PFFS) plans.
- It is your responsibility to verify our participation with your health insurance plan. We do not guarantee participation with your plan, or that services provided will be covered by your plan.
- It is your responsibility to provide us with complete insurance information and notify our office of any changes. Please present your insurance card at time of check-in for verification of coverage.
- It is your responsibility to inform us if your insurance carrier has a contract with a specific laboratory. If you do not alert us when your blood is drawn, you will be responsible for all incurred charges.
- You will receive two separate charges for radiology services performed at Pinnacle Medical Center. One charge will be from Pinnacle Medical Group. The second charge will be from the radiologist reading the exam.
- Failure to notify us 24 hours in advance if you need to cancel an appointment will result in a $25.00 charge. It is our commitment to notify you 24 hours in advance if we must reschedule your appointment. If we fail to keep our commitment to you, we will credit your account $25.00
- Medicare patients enrolled in certain HMO plans (Freedom Health, Universal Health Care, Wellcare) or any other non-participating plan will be considered a non-covered patient and will be responsible for payment at time of service.
