Dear Patient,

It is with fond memories and great regret to inform you of my retirement effective July 30, 2010. It has been an honor for me to care for your medical needs. I will leave with the feeling that we have accomplished a lot together, and I hope that you will continue to have a keen interest in your future health care needs. I will miss our visits very much.

I am leaving a group of very dedicated health providers that are ready to continue your care. Your medical records will remain here under the capable hands of Dr. Azim Lalani and Dr. Daniel Amoah, both fellow Family Medicine physicians. The office will ensure your care remains continuous and that your healthcare needs are met. Please contact the office by calling 941-792-2122 to schedule an appointment with either physician named above.

Of course, you may seek medical care from another physician. If you choose to do so, I recommend looking for a new physician as soon as possible. Your records are confidential and a copy can be transferred to another physician or released to you. You may request a records release form which will require your signature by calling 941-792-2122.

I have greatly valued our relationship and thank you for your loyalty and friendship over the years. Best wishes for your future health.
